That decline occurred in one week.

For context, testosterone typically declines by approximately 1% to 2% per year with age. A single week of insufficient sleep produced the hormonal equivalent of years of aging.

Get Tested in the Morning

Have your blood drawn before 10 a.m., preferably while fasting.

Testosterone typically peaks in the morning and declines throughout the day. An afternoon blood draw in an otherwise healthy man may appear abnormal.

Testing at the wrong time accounts for some men who believe they have low testosterone when they may not.

Confirm a Low Result

Any low result should be confirmed with a second morning blood draw.

Testosterone levels fluctuate. A single low value does not automatically establish a diagnosis, and therapy should not be started based on one isolated number.

Order the Full Hormone Panel

Total testosterone alone is not enough to explain what is happening.

A comprehensive hormone evaluation may include:

  • Total testosterone

  • Free testosterone

  • Luteinizing hormone (LH)

  • Follicle-stimulating hormone (FSH)

  • Sex hormone-binding globulin (SHBG)

  • Estradiol

  • Prolactin

LH and FSH help determine whether the problem originates with the signals from the brain or with the testicles themselves. This is one of the most important branch points in the evaluation.

Free testosterone and SHBG provide valuable context when total testosterone is borderline. Prolactin and estradiol help complete the picture and can occasionally reveal an unexpected cause.

A total testosterone result tells you whether one number is low.

It does not tell you why.

The cause determines whether you need lifestyle changes, a medication that helps preserve fertility, or testosterone replacement therapy.
